INSTRUCTIONS ON HOW TO FILL OUT THE QUESTIONNAIRE

MBF UNIVERSAL FITNESS TRAINING PROGRAM

Accuracy of the analysis of the current state of the body and physique will depend on the current filling out of the questionnaire on your part. Adequacy of the recommended diet, physical exercise programs and of suggested types of sports will depend on the flawless input of data into the questionnaire as well.

Attention! All the recommendations and comparisons which you will receive in the results of the filled in information is based on healthy adults. However, you cannot use them without consulting with a doctor.

Special warnings for people with pathological conditions, and those who are currently on prescription drugs:

NEVER UNDERTAKE AN EXERCISE OR DIET PROGRAM WITHOUT CONSULTING WITH YOUR PHYSICIAN!

Which indicators must necessarily be filled out in the questionnaire?

  1. First, last name or an abbreviation
  2. MONTH, DAY AND YEAR OF BIRTH, SEX, HEIGHT, WEIGHT, CIRCUMFRENCE OF BODY PARTS, BODY TYPE, HEART RATE in rest, BLOOD PRESSURE.
  3. Index of FITNESS LEVEL.
